Is the water leak in the pipework servicing the property, difficult to locate with little or no visual evidence of the leak? *Required field

Has the property on which the leak has occurred been subject to development, excavation, building, renovation, landscaping, concreting or construction within the previous six (6) months of when the leak occurred and/or was identified? *Required field

Has the leak been fixed within 30 days of identification/notification of the leak, or notification of higher than usual water consumption or the possibility of a leak? *Required field

Is the application being made within six (6) months of identification of the leak or notification of higher than usual water consumption or the possibility of a leak? *Required field

Have you got evidence including a plumber’s invoice or report from a licensed plumber or Home Assist, ACRO or Home Care, showing that the leak has been fixed? *Required field

Please note: If a licensed plumber has performed the repairs, please obtain a copy of the plumbing invoice or report. If the repairs were not performed by a licensed plumber, please provide a statutory declaration, appropriately witnessed, with the following details:

  • Who fixed the leak
  • What experience the repairer has to perform the works
  • The date of repair
  • Where the leak occurred on the property
  • How it was fixed
  • What materials were used to repair the leak
  • Any other details relevant to the leak repair

Has the property owner received a concealed leak financial adjustment on the same account within the last three (3) years? *Required field
